Dear Parents and Carers,
On Monday 23rd to Friday 27th February, we have a mobile Dental Van visiting onsite here at Ashwood School.
This is your chance to have your child seen by their friendly dental team.
Clinicians may offer the following treatments, depending on your child’s needs:
No treatment will be administered without the appropriate consent.
Once you have completed the consent form (sent home via Compass), the consent form can be emailed directly to The Dental Van at info@thedentalvan.com.au
If you prefer a hard copy form to complete, you can collect this from reception or request with your teacher for one to be sent home.
